Vitals Information: Death Certificate Records

Please find information regarding how to request a death certificate:

First, please complete a Death Certificate Request Form (PDF).

Funeral Home

If you are using a funeral home for arrangements, the funeral home will gather all of the information and enter it into the MA Registry of Vital Records and Statistics.

No Funeral Home

If you are not using a funeral home for arrangements, information will need to be provided to the Town Clerk to enter into the MA Registry of Vital Records and Statistics. Whomever is responsible for the arrangements will need to submit an Informant Worksheet for Certificate of Death (PDF). The attending physician will also need to submit a Death Certificate Medical Certifier Worksheet (PDF) unless they are able to submit it directly to the MA Registry of Vital Records and Statistics.
